Simply e-mail me w/ your: name, cell number and playing league, to ensure a fast sign up and I’ll add you to the list, then when I receive the completed application, AND a 20.00 deposit Check. via US mail you will be confirmed. You will receive your deposit check back at the Clinic on May 22nd, no shows will lose their deposit.

You Must be a D-4 coach to get this offer, after the first 50 coaches are signed up, or a non D-4 coach the price will be 40.00 each.
